A 70-year-old female presented with headache, confusion, seizures and was referred to the Olive Hospital Radiology department for MRI imaging. It was previously diagnosed as infection and the patient was not responding to any medication. After evaluating the MRI imaging of brain by its extensive involvement of more than 2 lobes of brain, patient has been advised contrast study & MR Spectroscopy/Molecular imaging (better characterisation of brain diseases by evaluating change in normal composition of brain chemicals). MRI brain contrast study revealed no abnormal enhancement & MRS revealed reversal of normal composition of brain chemicals (increased Choline: creatinine ratio & Choline: NAA ratio) in the abnormal areas, indicating Glial Tumor of brain; Adding to this DTI (Diffusion Tensor imaging) & Tractography showing destruction/loss of brain fibres in the abnormal involved areas.
Based on more than two lobes of brain involvement, diagnosis of the case was concluded as "Gliomatosis Cerebri". Giving his note on the occasion Dr. Sunil Bonela, MD, EDiR, Consultant Radiologist , said, 'Gliomatosis Cerebri (GC) is a rare, diffusely infiltrating glial brain tumor. Prognosis is poor, with 26-52% surviving less than a year from symptom onset. The 2007 World Health Organization (WHO) classification of central nervous system (CNS) tumors defined GC as an extensively infiltrative diffuse glioma involving at least three cerebral lobes. This review highlights our current understanding of GC and discusses recent 'molecular diagnostics' which may help in tailoring more efficacious therapeutic regimens.
